Position Overview:

Our service can provide the opportunity to work in a unique practice setting as we are the only home health program in Colorado that holds a Home Health license and a Community Integrated Healthcare Service license within a health service district. The Community Health Nurse will provide compassionate, high-quality care in the comfort of patients’ homes that empowers individuals to live healthier, more active lives in the beautiful outdoors of Eagle County, Colorado. The Community Health Nurse will have the opportunity to develop and implement individualized care plans, provide chronic disease management, work alongside EMS providers to respond to acute health issues in the home setting, teach patients and their families about managing various health conditions and have a lasting impact on improving the quality of life for individuals in our community. Our ideal Community Health Nurse will have substantial and specialized experience performing assessments, treatments and collaborative case management of home health services and will be experienced in nursing, with an emphasis on community health education/experience.
Ideal Candidate Profile:
- Required - Bachelor's degree in Nursing
- Preferred - Master's degree in Nursing
- Valid State of Colorado motor vehicle operating license
- Current licensure to practice nursing in Colorado (RN or LPN)
- CPR and First Aid Certification
- Minimum two years of experience, at least one of which is in the area of public health or home care nursing is preferred.
- Ability to effectively communicate in written and verbal form with caregivers of other disciplines, community agencies, physicians, and patients/families.
- Strong clinical skills and knowledge of medical terminology and home health care practices.
- Strong organizational skills with attention to detail.
- Ability to remain calm under pressure and manage emergency situations.
Compensation and Benefits:
- Hourly rate: $41-$55 (based on experience and qualifications)
